Telehealth includes the practice of psychological health care delivery, diagnosis, consultation, treatment, referral to resources, education, and the transfer of medical and clinical data.

Insurance.
Question: What type of insurance do you accept?
Answer: Stacy accepts several major insurance plans (United Healthcare, Blue Cross Blue Shield BCBS, Aetna, and several others) as well as credit card payments for counseling, consultation and group services.
We can provide you a Superbill that you may be able to submit to your HSA for reimbursement or to your insurance company. We can also provide an invoice if you are being reimbursed or paying through your company/employer. We can assist you in submitting documentation for reimbursement.

Confidentiality & Record Keeping.
We use Simple Practice as our Electronic Health Portal. Records are stored electronically and are secure. We follow HIPPA guidelines and below are our statements on our policies and procedures that outline how we approach confidentiality.

What if I want to request my records?
To obtain/authorize us to send a copy of your records, please submit a release of information (ROI) to our administrative email address at [email protected]. The form may be requested from your clinician or downloaded below.

Do you treat minors and what do I need to know?
We are happy to work with your children and adolescents. Please note that if parents are not married, are legally separated, or divorced, or if there is a child custody agreement, we will require a copy of your legal paperwork before beginning treatment. If parents share legal decision making, we are legally required to notify both parents of the minor child’s enrollment in treatment and will not continue treatment if either parent withdraws consent for treatment. Parents will be required to provide the contact information for the other parent at the beginning of treatment so that we may communicate the minor child’s enrollment in therapy.
What if I have a complaint?
If you have a complaint we prefer that you address your concern directly with your therapist. If you are unable to resolve the issue please email your concern to [email protected].
